There is also a $40.00 fee that must be paid. Students and parents must view the video Know the Risks and sign a waiver. Parents need only view the video once while athletes must view it each year. A health history screening and physical examination is required EVERY TWO YEARS, unless the Health History Screening indicates need for more frequent examinations. Physical Form B (pdf document) File Size: 60.26 kb This form must be completed prior to participation in NDHSAA athletic events. Along with a physical examination on file at the school this Health History Screening is required in years alternating the mandated physical examination. The parent or guardian is accountable to provide accurate information. This information will determine the need for a repeat physical.
